We are a small group of clinicians who offer a personalized approach to the specialized needs of the older adult, their loved ones and caregivers.
Our social workers and psychologists are experienced in home health care and offer a strength based approach that integrates the collaboration of physicians, facility staff, family members and ancillary services in treatment planning and coordination of care.
We visit with seniors and their families in the comfort of their own home or in assisted living facilities. We are also available in our confidential offices in Lake Zurich.
Our goal is to compassionately partner with seniors to help support a life that is worth living.
